Job Description:
As a clinical leader, you will oversee the Acute Frailty Team within Monash Health Emergency Departments. Your primary objective is to deliver high-quality, evidence-based care for older adults presenting to ED.
* Streamline patient flow and facilitate timely access to appropriate care pathways.
* Educate ED staff on best practice for frailty and delirium management, falls risk mitigation, and person-centred care.
Key Result Areas:
The ideal candidate will have excellent leadership skills, with experience in managing teams of healthcare professionals. Strong communication skills are essential for effective collaboration with colleagues across departments.
