As a compassionate and dedicated Community Care Coordinator, you will play a vital role in assisting clients with their care management needs. Your key responsibilities will include working closely with support coordinators, allied health professionals, and clinical teams to develop and refine client goals.

Responsibilities:
* Developing and refining client goals in collaboration with local support coordinators, allied health professionals, and clinical teams.
* Providing information and support to clients and their families/carers to monitor the use of their funds.
* Ensuring accurate case noting, service agreements, and client reviews that reflect forward planning in line with funding plans.
* Conducting annual reviews for all clients, revising care plans, goals, service provisions, and budget expenditure.
* Ensuring service provisions are conducted ethically, fairly, and within statutory legal and contractual requirements.

Requirements:
* AHPRA Registration.
* Understanding of NDIS and Home Care Packages.
* Previous case management experience.
* Demonstrated knowledge of community care funding models.
* Exceptional customer service, negotiation, and influencing skills.
* Excellent ability to connect with multiple stakeholders and think commercially.
* Strong interpersonal skills and high levels of resilience.
* Driver's license and access to own vehicle, company car available for use in business hours.
* Professional verbal and written communication skills.
