Position Classification: Clinical Nurse Specialist Grade 2
Remuneration: $116,639.97 - $120,469.88 per annum
Hours Per Week: 38 (Job Share may be considered for part time hours)
Location: Mount Druitt Community Health Centre

You'll join a dedicated Aboriginal Chronic Care Program that values cultural respect, collaboration and clinical excellence, working alongside passionate professionals who are committed to equity and closing the gap.

· Work within a supportive Aboriginal Health and Integrated Community Health team

· Partner closely with Aboriginal communities and local services

· Deliver care in a culturally safe, inclusive and strengths-based environment

· Contribute to innovative models of chronic and complex care

· Be part of a service that prioritises advocacy, respect and community voice

What you'll be doing
As the Aboriginal Chronic Care Nurse (CNS2), you will provide advanced clinical leadership in chronic and complex care management, ensuring services are culturally appropriate, coordinated and responsive to community needs.

· Lead and advance specialty nursing practice for Aboriginal clients with chronic and complex conditions

· Coordinate planned care, case management and multidisciplinary collaboration

· Advocate for Aboriginal clients, families and carers to improve equity and access

· Drive quality improvement, clinical innovation and best-practice service delivery

· Mentor and support nursing staff while contributing to service development and research initiatives

This role requires strong clinical expertise, cultural capability, leadership skills and a genuine commitment to improving health outcomes for Aboriginal and Torres Strait Islander communities.

Essential Criteria:

· License: Current Drivers Licence valid within Australia

· Qualification: Post registration graduate qualifications relevant to performing the role of Clinical Specialist Grade 2 for Aboriginal Chronic Care e.g. Advanced Nursing and/ Aged Nursing, Critical Care Nursing and /or Diabetes Management, and /or Renal Nursing or equivalent

· Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)

Selection Criteria:

1. Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA) with recent postgraduate clinical nursing experience three (3) years full time equivalent post registration experience.

2. Demonstrated extensive experience working with Aboriginal people with chronic and complex health issues and their families.

3. Post registration graduate qualifications relevant to performing the role of Clinical Specialist Grade 2 for Aboriginal Chronic Care e.g. Advanced Nursing and/ Aged Nursing, Critical Care Nursing and /or Diabetes Management, and /or Renal Nursing or equivalent.

4. Demonstrated skills and knowledge in case management, including care coordination and education on self-management for Aboriginal people living with complex and chronic conditions, using culturally appropriate and advanced clinical skills in patient/client Assessment and care planning.

5. Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research and best practice organisations.

6. Demonstrated ability to communicate effectively with Aboriginal clients, carers and a diverse range of health care providers including those in government, non-government and community

7. Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based services.

8. Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training.
