Job Title: Care Coordinator
We are seeking experienced and qualified Care Coordinators for short-term placements in aged care and community settings across rural and remote regions of Australia.
* Our ideal candidate will provide person-centred care and support to clients with a variety of needs, including those with dementia and high care requirements.
* They will develop, coordinate, and implement care plans tailored to individual client goals.
* Responsibilities include supporting clients with mobility, safe operation of hoists and other manual handling equipment.
* The successful candidate will collaborate with families, carers, and multidisciplinary teams to ensure holistic care delivery.
* Maintenance of accurate, timely documentation is essential in line with organisational and regulatory standards.
* Promoting a safe, inclusive, and culturally respectful environment for clients and colleagues is paramount.
Requirements
* A Certificate III (or higher) in Aged Care, Home and Community Care, or Disability.
* Qualified and experienced Care Coordinator, with a minimum of three years' experience working in community or residential aged care.
* Demonstrated knowledge and/or recent experience in aged care, including dementia care and manual handling (hoist operation).
* A current Australian driver's licence, and access to a registered vehicle with a minimum of third-party insurance.
* Current or recent employment experience as a Care Coordinator.
* A current Police Check and Working with Vulnerable People (WWVP) registration, or willingness to obtain prior to placement.
