About the Role
This position plays a critical role in the multi-disciplinary Offender Services team. The Assessment and Transition Co-ordinator is responsible for the administration of the Corrections Victoria Reintegration Pathway, including co-ordination and facilitation of prisoner risk and needs assessments, transition and discharge planning, parole application processes, and referrals to a range of pre and post release services.
Key Responsibilities
* Support the delivery of the Corrections Victoria Reintegration Pathway in a prison environment.
* Administer prisoner risk and needs assessment tools to effectively identify rehabilitation and reintegration requirements, making referrals to relevant programs and services as required.
* Work collaboratively with multi-disciplinary teams to develop integrated and effective transition and other specialist support plans, including actively involving and motivating prisoners to engage in goal setting and program/service participation.
Requirements
* Demonstrated understanding of the specific transition and reintegration issues and needs of prisoners.
* Demonstrated ability to administer risk and/or welfare assessment tools.
* Effective stakeholder management skills.
* Ability to communicate sensitively and effectively with diverse cohorts, especially Aboriginal and Torres Strait Islander communities.
* Demonstrated capability to work with people who are difficult to engage individually, and in group settings.
