Job Title: Transition and Employment Facilitator
About the Role:
* Engage and empower a caseload of individuals at risk of long-term unemployment, disengaged from education, or with multiple barriers to employment.
* Motivate and guide participants in developing skills and attributes for sustainable career progression, including apprenticeships and traineeships.
* Facilitate connections with education, training, and labour market opportunities for long-term employment outcomes.
This role requires strong interpersonal, organisational, and communication skills, as well as the ability to work independently and collaboratively in a team environment.
Key Responsibilities:
1. Develop and implement individualised plans to support participants in achieving their employment goals.
2. Maintain accurate records and reports on participant progress and engagement.
3. Collaborate with stakeholders to identify and secure job opportunities and training programs.
