We are seeking an experienced Care Coordinator to join our team. As a Care Coordinator, you will be responsible for coordinating, managing and administering the delivery of care services to our clients.
Responsibilities
* Manage a client load of 30-40 clients.
* Be the primary point of contact for our clients for everything from rostering, budgets, purchases, and more.
* Provide guidance and technical advice to care staff.
* Lead within our organization and be a role model for the company.
* Offer nursing support to our clients (for ENs and RN applicants only).
* Advocate for your clients for all matters.
Requirements
* Cert III/Cert IV in Aged/Disability Care or Registered Nurse (RN) with current registration or Enrolled Nurse (EN) with current registration.
* Experience within aged care/disability/mental health in a community setting.
* Understanding of aged care & Disability standards and consumer-directed care.
* First Aid, current CPR, and National police check.
* Working with children Blue Card or willingness to obtain.
* Drivers license - Australia.
* High level of computer literacy and understanding of technology.
* Understanding of clinical governance and continuous improvement.
* Possess a 'can-do' positive attitude and work well in a team environment.
* Able to problem-solve and think quickly under pressure.
* Able to multitask with a strong attention to detail while keeping calm and focused.
* Excellent customer service skills with exceptional verbal and written communication skills.
