About the Role
Join a supportive team delivering social work services to veterans and their families, with a strong focus on health coordination and continuity of care. Working within a multidisciplinary team, you will play a key role in coordinating care plans and ensuring a seamless, client-centred experience across services.

Key Responsibilities
* Coordinate and manage care for veterans, ensuring access to appropriate health and support services.
* Conduct assessments, develop care plans, and support ongoing case management.
* Work closely with GPs and allied health professionals, as well as the wider care team to deliver integrated care.
* Monitor client progress and adjust support plans to optimise outcomes.
* Maintain accurate and timely documentation.
About You
* Qualified Social Worker with current AASW membership (new graduates encouraged to apply).
* Experience or strong interest in care coordination and integrated service delivery.
* Veteran or DVA experience desirable.
* Strong communication, organisation, and stakeholder engagement skills.
* Comfortable delivering services via face-to-face and telehealth platforms.
* Passionate about improving health outcomes through coordinated care.
