The Care Connect Worker is a key member of the team, responsible for engaging with and supporting clients experiencing social and emotional wellbeing issues at the Logan Social Health and Primary Health Clinic. This role will support the needs of families and individuals by operating within a strengths and priority-based framework.

Key Responsibilities include:

* Providing social emotional wellbeing support to clients

* Assisting clients to address practical issues relating to their SEWB needs

* Supporting the coordination of care and managing data

* Committing to high quality management through the use of LogiQC and MOBnet quality management systems

* Cultivating productive working relationships and developing staff

To be successful in this role, applicants must possess:

Requirements
">
Applicants must have:
">
* A minimum Certificate III in Community Services, Mental Health or Disability, or three years of equivalent professional experience
">
* Demonstrated ability to work with Aboriginal and Torres Strait Islander communities and their leaders
">
* Demonstrated experience in facilitating positive behaviour change with individuals and families
">
* Demonstrated ability to work effectively with minimal direction and communicate collaboratively as part of a multi-disciplinary team
">
* Demonstrated IT capability, including experience in computer-based data management systems and Microsoft Office
">
* A willingness to provide supports and engagement outside of a clinic environment
">
* Working with Children's Card (Blue Card) and NDIS Worker Screening (Yellow Card), or eligibility to apply
">
* Current C Class Drivers Licence (Qld) and ability to travel daily around SEQ
">
