From 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health have merged to form a new, unified not‑for‑profit community health organisation, Omnia Community Health. This decision follows an extensive exploration process that confirms the merger improves access to safe, high‑quality and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.
The new organisation brings together the deep community connections, shared values and specialist expertise of both entities. Services span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding communities.
About the Role
This role is accountable for the end-to-end implementation of a new client management system (CMS), from solution scoping and procurement through to system configuration, data migration, staff training and go live. Working closely with the Manager ICT and Data, Executive and key stakeholders, you will ensure the transition from legacy systems is well-planned, well-governed and delivered safely.
The role is informed by consultation and discovery outputs provided by a dedicated CMS requirements consultation lead, allowing you to focus on delivery, execution and outcomes.
Key Responsibilities
* Lead the CMS implementation within a structured project management framework.
* Own solution scoping, RFQ development, procurement and vendor engagement.
* Coordinate assessment processes, vendor presentation and recommendations to Executive and Board.
* Manage project timelines, risks, dependencies and implementation milestones.
* Oversee system configuration, integration, data migration, testing and validation.
* Plan and coordinate staff training, change management and implementation support.
* Work with managed service providers and vendors to ensure appropriate technical expertise and governance.
* Provide regular project status reporting, including risks, budget position and progress updates.
What you Bring
* Demonstrated experience leading the implementation of complex ICT systems, ideally in health, community services, aged care or disability settings.
* Strong capability in system scoping, procurement processes and vendor management.
* Proven experience in data migration, system testing and go live transitions.
* Excellent stakeholder engagement skills, with the ability to work effectively with executives, managers, frontline staff and external partners.
* A structured approach to change management and staff readiness.
* Strong communication skills, with experience preparing reports and recommendations for senior leadership and Board.

Location
The role can be primarily based at our Kyneton or Broadford site, with travel across other sites occasionally required.
