About the Role:

We are seeking a skilled Home Community Support Worker to provide high-quality, person-centered support to individuals with disabilities. As a key member of our team, you will play a crucial role in helping our clients achieve their goals and improve their physical and mental health.

Key Responsibilities:

* Providing individualized support services tailored to meet the unique needs of our clients, including housekeeping, grooming, toileting, meal preparation, medication management, and more.
">
* Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans.
">
* Assisting clients in participating in community activities and services based on their preferences and abilities.
">
* Providing regular feedback to the Home and Community Support Manager regarding issues that require follow-up.
">
* Adhering to our Code of Conduct, policies, and Quality Management System procedures to ensure the highest standards of care and support.
">
* Demonstrating a strong commitment to the safety and well-being of yourself, clients, and others in the work environment.
">
">
About You:
">
To succeed in this position, you will need:
">
">
* A Certificate III or IV in individual supports (aged care, disability, or home and community services).
">
* A valid driver's license and vehicle.
">
* A current Working With Children Check.
">
* Experience supporting individuals with Autism.
">
* Experience working within the In-Home Care environment.
">
* The ability to meet deadlines, work with minimal supervision, and manage pressure.
">
* Effective written and verbal communication skills, as well as the ability to write clear case notes.
">
* An understanding of Workplace Health and Safety procedures.
">

Desirable Qualifications:
">
In addition to the above qualifications, we also welcome applicants with experience in community services, disability, mental health, or related fields.
