About the role:
Working across Melbourne, you will be supported by a dedicated Care Manager and experienced team, to assist our clients to remain living independently in their own homes. We establish tight-knit care teams by endeavouring to match clients with carers and nurses who have compatible interests to support our clients.
In this role, you will support our clients and their carers by providing direct clinical care, as well as support, advice and direction on a wholistic care delivery model that includes but is not limited to:
* High level clinical care;
* Wound management;
* Medication Management;
* Catheter management;
* Physical and cognitive assessments;
* Chronic disease management (diabetes, respiratory, cardiac);
* Palliative care management;
* Ensuring our clients receive the best possible individualised care and support, through building strong, professional relationships.
Available employment/shifts:
* Permanent full time and/ or part time positions available based in Hawthorn area
* Base rate of $42.65 per hour for a Enrolled Nurse + penalty rates on weekends and public holidays
* Base rate of $50.28 per hour for a Registered Nurse + penalty rates on weekends and public holidays.
* Shifts are available across Melbourne
* A core requirement of this position is maintaining availability between 7am and 7pm on four (4) days per week.
We are looking for people who have:
* A current registration with AHPRA as either a Division 1 Registered Nurse or Division 2 Endorsed Enrolled Nurse.
* Endorsed Enrolled Nurses with at least 2 - 4 years experience in the field.
* Registered Nurses with at least 5 years experience in the field.
* A passion for caring for people and delivering a high standard of care.
* Excellent communication, customer service and interpersonal skills.
* Exceptional organisational and problem-solving skills.
* Access to a smartphone.
* A current driver's license and access to a registered, insured and well-presented car.
* The right to work in Australia & can obtain a satisfactory Police Check.
* Prepared to work flexible hours and travel throughout the Mornington Peninsula.
