**About Us**
Berry Street's Home Based Care team in the Hume Region provides a Foster Care service, which aims to meet the needs of children and young people who are unable to live at home with their family for periods of time. The foster care case manager will be primarily responsible for supporting foster carers and working collaboratively with the care team to support the best interests of each child/young person placed in care.
The Home Based Care team are currently searching for a Foster Care Case Manager to join the team. The role can be based in our Shepparton or Seymour office and you will play a key role in working with and supporting children/young people placed in foster care, their families and foster carers.
**Key Responsibilities**
- Demonstrated knowledge and practice experience in the child welfare field.
- Ability to provide statutory case management services to clients placed in foster care.
- Provide leadership and coordination of the care team
- Demonstrated understanding of Children, Youth & Families Act 2005.
- Liaising with stakeholders including DFFH, external agencies, education, allied health services and other professionals.
- Working directly with and supporting young people and their birth families where required,
- Provide supervision and support to foster carers.
**About You**
You will also have;
- Experience in the child and family welfare field supporting and maintaining interactive connections with family/carers and the community.
- Demonstrated ability to provide high quality, innovative direct service in the assessment and treatment of children, young people and families.
- Knowledge and experience in the use of current and relevant theoretical frameworks in relation to working with Berry Street's approach to Out of Home Care.
- Providing information on procedures, guidelines and appropriate legislation.
