This role involves collaborating with clients, general practitioners, clinic staff and other health service providers to deliver care services for individuals with chronic conditions and enhance health outcomes.
* Supporting clients with chronic health conditions residing in remote areas
* Assisting clients requiring travel to Alice Springs or interstate for specialist appointments
To be considered for this position, applicants will require:
1. A registered nurse or Aboriginal Health Practitioner with at least 2 years of clinical experience in chronic disease management and/or primary healthcare
2. An excellent understanding of primary healthcare and sound clinical knowledge of chronic disease management
3. An understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control
