Care Coordinator Role
This pivotal role is central to the delivery of patient care, ensuring optimal health outcomes for elderly clients in the community. The care coordinator will maintain a caseload including comprehensive psycho-social assessments, care planning, risk assessment, education, advocacy, counseling, liaison with community agencies and relevant parties.
The role requires a level of autonomy in problem-solving without direct clinical supervision when completing in-home assessments and interventions. Ideally, this position would suit someone with at least 2 years' experience working in a similar setting.
The position functions as part of an interdisciplinary team providing home-based intervention to frail and often socially complex older clients who require additional care and support.
Key Responsibilities:
* Conduct comprehensive psycho-social assessments and develop individualized care plans
* Collaborate with multidisciplinary teams to ensure seamless care coordination
* Provide education, advocacy, and counseling to clients and their families
* Facilitate communication between clients, healthcare providers, and community agencies
* Monitor client progress and adjust care plans as needed
Requirements:
* Minimum 2 years' experience working in a similar care coordination role
* Strong understanding of community resources and services
* Excellent communication and interpersonal skills
* Ability to work autonomously and as part of a team
