Job Summary:
Our organization is seeking a skilled Registered Nurse to provide high-quality care to our clients in the Ballarat and surrounding communities. The successful candidate will be responsible for conducting in-home clinical assessments, providing clinical advice and support, and working closely with local allied health professionals.
Key Responsibilities:
* Conduct in-home clinical assessments and reviews of client needs
* Provide clinical advice and support to clients and refer them to specialized services when necessary
* Lead and supervise Care Advisors and Home Care staff
* Collaborate with local allied health professionals to deliver responsive care in client homes
* Deliver health education and promotion through social support groups and carer education
* Promote a harmonious team approach by setting a positive example in coordinating daily routines and adhering to safe practice measures in medication management
Requirements:
* Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration
* Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing
* Team player with a flexible approach to work and a positive outlook
* Endorsed for Continence Assessments and Prescription (highly desirable)
* Experience in actively visiting clients in their homes and evaluating and implementing care support plans
* Familiarity with the My Aged Care portal and proficiency in decision making, critical analysis, and on-the-spot problem solving
* Demonstrated ability to delegate and prioritize in a fast-paced environment with excellent communication skills in team and one-on-one interactions
