We are seeking a compassionate and organised individual to manage relationships and oversee service compliance for a portfolio of clients.
The successful candidate will be responsible for completing client assessments and care plans, mitigating client challenges and facilitating a smooth home care experience.
Key Responsibilities
* Complete thorough client assessments and develop tailored care plans
* Mitigate potential risks and ensure smooth service delivery
* Supervise the provision of care services to clients
* Review client progress notes and implement corrective actions as necessary
* Meet with prospective clients to explain the value proposition of our service
* Report and manage any incidents or hazards that may arise
* Develop strong relationships with clients and their families, seeking feedback to improve service delivery
* Collaborate with scheduling teams to ensure staff and client expectations are met
* Chair regular team meetings to support staff in their duties
* Conduct performance reviews with team members
Required Skills and Qualifications
We are looking for a passionate and motivated professional who is AHPRA registered (Nurse, Occupational Therapist or Social Worker). The ideal candidate will have excellent communication and interpersonal skills, with the ability to work collaboratively as part of a team.
A willingness to learn and adapt to changing circumstances is essential, as well as a commitment to providing high-quality service to our clients.
