Our team are located primarily at Royal North Shore, Mona Vale and Hornsby. This role will be located at Royal North Shore Hospital campus.What You'll Be DoingUsing the HITH model of care to provide a framework of acute care, delivered at home. You will be working with a multidisciplinary team that includes nursing, medical, physiotherapy, occupational therapy, pharmacy and with another social worker.People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.Candidates Will Need To Meet The Following CriteriaA Bachelor or post graduate degree in Social Work with eligibility for the Australian Association of Social Workers (AASW). Level 2 Social Workers must show evidence of minimum three years’ experience in a health care setting working as a qualified Social Worker.Demonstrated understanding of Hospital in the Home model of care. Demonstrate skills and knowledge of a Social Worker in the provision of service to consumers, carers, and families requiring assessment, counselling, crisis management, advocacy, discharge planning, and appropriate referrals.Proven ability to work independently with minimal supervision and as a collaborative member of the multidisciplinary team, in a health care environment, with competing demands.
