About the Role
As a HCP Coordinator (Clinician), you'll play a vital role in providing holistic, person centred clinical and care coordination to older people living in the community under the Home Care Package program.
Working as part of a supportive multidisciplinary team, you'll manage your own caseload of consumers, coordinating assessments, developing and monitoring care plans, and ensuring clinical compliance aligned with the Aged Care Quality Standards. This role combines autonomy and collaboration, with the chance to make a real impact in people's lives every day.
Key Responsibilities
* Conduct clinical assessments, care planning and ongoing reviews for HCP consumers.
* Coordinate services and supports that promote wellbeing, independence and safety.
* Provide clinical leadership and guidance to non-clinical team members.
* Maintain accurate documentation and compliance with all quality and funding frameworks.
* Collaborate with allied health, support workers and families to ensure quality outcomes.
About You
You'll be a compassionate and organised clinician who values teamwork, professionalism and care. You thrive in a dynamic environment, manage your time effectively, and approach change with a positive, solutions focused mindset.
Essential Criteria
* Current AHPRA Registration – Enrolled Nurse (EEN) or Registered Nurse (RN).
* Minimum Diploma in Nursing (EEN) or Bachelor of Nursing (RN).
* First Aid & CPR certification (ongoing renewal required).
* Demonstrated experience in aged or community care (or transferable clinical skills).
* Strong assessment, documentation and time management skills.
* Confident using clinical systems and Microsoft Office.
* Current NSW Driver's Licence and reliable vehicle.
Desirable
* Catheter care competency.
* Knowledge of Home Care Package (HCP) or community funding frameworks.
