What You'll Do
Office-based role providing case management and support via phone and email to a dedicated portfolio of clients
Partner with clients to create, review, and adjust personalised care plans – ensuring they understand their options and make the most of their package
Build trusted relationships with clients, families, and providers by navigating complex issues and sensitive situations with professionalism and care
Confidently handle challenging conversations, guiding clients towards positive decisions that improve their independence and quality of life
Provide clear education around care budgets, helping clients see opportunities to access more services and get the best value from their package
Proactively identify risks and opportunities to enhance client outcomes, ensuring safe, high-quality care supported by accurate documentation
Contribute to a positive team culture through shared learning, collaboration, and a proactive approach to client success
What You'll Bring
Customer service or call centre experience (case management, client support, or account management highly regarded)
Confidence managing fast-paced, phone-based interactions with empathy, professionalism, and influence
Strong communication skills, including the ability to educate and guide clients through complex conversations
A proactive approach to uncovering opportunities that help clients make the most of their care package
Calm, solutions-focused mindset when challenges arise
Comfortable using multiple systems, including Microsoft 365 and CRMs, with the ability to quickly learn and adapt to new technology and digital tools
Organised, adaptable, and able to manage competing priorities in a dynamic environment
