The RAS is responsible for providing Home Support Assessments to assess people for eligibility for entry level supports that will enable older people to continue living independently in their home. RAS is part of the broader department with ACAS & TCP.
**About The Role**
The Home Support Assessor provides phone, telehealth and in home (face to face) assessments of clients referred via the My Aged Care (MAC) Gateway using the National Screening Assessment Form (NSAF). The NSAF will be transitioned to the Integrated Assessment Tool (IAT) in line with the aged care reform to a Single Assessment Service. The Home Support Assessor completes the NSAF (or IAT) and matches and refers clients to appropriate service(s). Home Support Assessors may also follow specific clients over a period of time if they require some coordination of service linkage and have re-ablement needs.
**About You**:
- Exceptional and effective communication skills with great attention to detail.
- Able to work collaboratively within a team environment while also being able to work autonomously.
- Knowledge of the healthcare, aged care and community setting and advanced clinical assessment skills.
**What You Need**:
- Tertiary qualification eg. Aging Support Cert IV, Individual Support (Ageing) Cert III, Community Services Work cert III or IV, Community Services Cert III or IV, Community Services Advocacy Cert III or IV
- Current Australian driving licenses
- Australian working rights
