What You'll Do
In this role, you'll continue to deliver high-quality physiotherapy services while stepping into Care Management responsibilities within our Restorative Care Pathway. It's a fantastic opportunity to move sideways from direct clinical care and take on a more coordinating, goal‐focused position. You will also:
* Deliver personalised, evidence-based in-home physiotherapy services for older adults.
* Facilitate group exercise and healthy lifestyle programs to promote strength, balance, and overall wellness.
* Coordinate clinical services and Care Management activities as part of the Restorative Care Pathway, ensuring clients receive the right supports at the right time.
* Work closely with clients to identify goals, develop restorative plans, and overcome barriers to independence.
* Collaborate with multidisciplinary teams to ensure holistic, person‐centred care.
What We're Looking For
* A registered Physiotherapist (AHPRA) who thrives in a community care setting
* Strong organisational and communication skills.
* Someone excited to broaden their scope into Care Management and service coordination
* Comfort using technology, including MS Office and client management software.
* A valid driver's license and a willingness to travel
