Offer description

The Clinical Care Coordinator will work and promote the use of a single multiagency health care plan to deliver focused interventions for individuals with severe and complex mental health needs in a primary care setting.

The position will be responsible for providing brief individual/group clinical therapeutic services to clients within Port Lincoln and surrounding areas. Provide evidence-based therapies for people with severe mental health needs. The priority of Clinical Care & Coordination is planning and improving physical health and wellbeing for people living with mental illness.

What you will bring to the role

* Formal tertiary qualifications in Nursing, Psychology, Social Work, Occupational Therapy or related
* Formal qualifications in the following disciplines: Mental Health Social Work or Mental Health Nursing (or working towards accreditation to become a Mental Health Social Worker)
* Accredited registration with a professional governing body such as AHPRA, ACA, AASW
* Experience in the area of mental health
* Experience in delivery of evidence-based brief interventions
