About the role
We are seeking a highly motivated and compassionate Lead Practitioner - Family Engagement for a fixed-term, part-time (30.4 hours per week) opportunity at our Youth Step Up Step Down residential service in Caboolture. In this role, you will advocate and work collaboratively with a team to enable the best outcomes for young people, ensuring that families and carers are supported and included in the services delivery.
You will also provide practice leadership, guiding staff in delivering psychosocial rehabilitation, recovery programs, and support services in line with Mind's Recovery-Oriented Practice and values.
About the service
The Youth Step Up Step Down (YSUSD) service is a short-term residential mental health program for young people aged 16–21, delivered in partnership with the local health service. It provides intensive individual and group support to build resilience, social connection, and wellbeing. YSUSD offers a 'step up' alternative to hospital admission or a 'step down' transition from hospital back to the community.
Key responsibilities
* Provide direct support to young people by developing individual recovery plans, enhancing daily living skills, and fostering independence.
* Deliver family engagement work by supporting families and carers, providing education, facilitating connections, and ensuring their inclusion in service planning.
* Guide and mentor Community Mental Health Practitioners through coaching, training, and professional development to foster a high-performing, collaborative team.
* Plan, develop, and deliver group programs to help young people build skills, focus on recovery, and transition back to the community.
* Collaborate with local service providers to connect young people with appropriate supports, including mental and physical health services, education, employment, housing, and social opportunities.
What you'll bring
* Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
* Hold a current First Aid and CPR certification.
* Proven experience working in family engagement support roles in the mental health or community services sectors.
* Strong understanding of trauma-informed, recovery-oriented, and family-inclusive practice.
* Ability to provide a record of vaccination for Vaccine Preventable Diseases or willingness to obtain vaccinations for Measles, Mumps, Rubella, Chickenpox, Whooping Cough, Hepatitis B, or other diseases as required through our partnership with QLD Health.
