About the Opportunity
Are you passionate about making a real difference in patients' lives? Join our dynamic Community Nursing team, where you'll provide essential care in homes and clinics—improving health outcomes and helping to prevent unnecessary hospital admissions.
As a Registered Nurse, you will provide safe, compassionate, high-quality, person-centred care. You'll collaborate with patients, families/carers, and a multidisciplinary team to achieve shared health goals and outcomes. You'll also work closely with the Palliative Care, HITH (Hospital in the Home), and Wound Care teams to ensure comprehensive patient management.
The role offers flexible morning and afternoon shifts on a 7-day rotating roster, including weekends and public holidays.
You'll be supported by a skilled, diverse, and dedicated team, including the NUM, CNCs, and CNE, who are committed to your growth and success.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

1. Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA).
2. Demonstrated recent experience as a Registered Nurse in an environment appropriate to the relevant speciality.
3. Demonstrated ability to provide safe compassionate quality care that is culturally responsive.
4. Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
5. Demonstrated capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise.
6. Demonstrated evidence of the use of critical thinking as it relates to the delivery of evidenced based nursing care.
7. Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace.
8. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
