About the Role
We are seeking an experienced Care Coordinator to lead and coordinate safe, person-centred care for participants with complex needs across disability, aged care, and community health programs.
This role plays a pivotal leadership function, combining clinical oversight, care coordination, workforce supervision, and operational accountability. You will act as the primary point of contact for allocated participants while providing line management and professional support to Disability Support Workers.
This position reports to the Clinical Services Manager – Aged Care & General Health / Mental Health & Behaviour Support and is based in Karratha.
Key Responsibilities
* Develop, implement, and review individualised Care and Support Plans aligned with participant goals, clinical best practice, and NDIS requirements
* Coordinate multidisciplinary care, including liaison with allied health, medical practitioners, behaviour support practitioners, plan managers, and families
* Complete End of Plan Reports within required timeframes and monitor participant progress against agreed outcomes
* Provide line management and supervision to Disability Support Workers, including supervision, performance reviews, competency assessments, and rostering
* Maintain high standards of clinical documentation, compliance, and reporting using care management systems
* Monitor service delivery quality through audits, incident management, and continuous improvement activities
* Undertake limited direct care shifts to maintain participant rapport and clinical oversight
* Ensure compliance with NDIS Practice Standards, Aged Care Quality Standards, restrictive practice governance, and WHS requirements
* Contribute to service planning, workforce capability, and sustainable growth initiatives
Essential Qualifications & Requirements
* Bachelor of Nursing with current unrestricted AHPRA registration
* Minimum 2 years' experience in care coordination, senior nursing, or team leadership within disability, aged care, or community services
* Demonstrated experience supporting participants with complex health, mental health, or behavioural support needs
* Strong understanding of NDIS Practice Standards, restrictive practice requirements, and person-centred care principles
* Experience supervising, mentoring, and supporting frontline support staff
* Proficiency in care management systems and Microsoft Office
* Current First Aid and CPR certification
* NDIS Worker Screening Clearance and National Police Check
* Current Australian Driver's Licence and access to a reliable vehicle
Desirable (but not essential)
* Certificate IV in Leadership and Management
* Certificate IV in Training and Assessment
* Experience working in culturally diverse or regional and remote communities
* Training or experience in Positive Behaviour Support

Pay: $95,000.00 – $105,000.00 per year
