Position Purpose
The Disability Support Worker is responsible for providing high-quality, person-centred support to participants in line with their individual goals, care plans and NDIS funding. The role focuses on promoting independence, dignity, safety and inclusion in all aspects of a participant's life.
Key Responsibilities
Participant Support
• Provide support with daily living tasks including personal care, hygiene, dressing and meal preparation
• Assist with household tasks such as cleaning, laundry and organisation
• Support participants to attend appointments, community activities and social events
• Assist participants to develop life skills such as cooking, budgeting, shopping and communication
• Follow individual support plans, behaviour support plans and risk management strategies
• Encourage choice, control and independence in all areas of support
Health and Safety
• Monitor participant wellbeing and report any concerns or changes to supervisors
• Follow medication support procedures where trained and authorised
• Use safe manual handling techniques and follow workplace health and safety procedures
• Maintain participant privacy, dignity and confidentiality at all times
Documentation and Communication
• Complete accurate and timely progress notes using the organisation's online system
• Report incidents, hazards or concerns in line with RISE policies and procedures
• Communicate effectively with participants, families, team leaders and other professionals
• Attend team meetings and training as required
Professional Conduct
• Work in line with the NDIS Code of Conduct and organisational policies
• Maintain professional boundaries at all times
• Represent RISE in a positive and professional manner
• Participate in ongoing training and professional development
Selection Criteria
Essential
• Genuine interest in supporting people with disability
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Reliable, responsible and professional attitude
• NDIS Worker Screening Check (or willing to obtain)
• Working With Children Check
• Current First Aid and CPR
• Current driver's licence and reliable vehicle
• Smartphone for rostering and documentation apps
• Minimum of 6 months experience in disability support or related field
• Australian citizenship required for this position
• All required checks must be obtained prior to commencing employment

Desirable
• Certificate III or IV in Disability or Individual Support
• Previous experience in disability, aged care or community services
• Experience supporting people with complex needs or behaviours of concern
• Manual handling training
Working Conditions
• Casual position with flexible shifts including weekdays, evenings and weekends
• Work takes place in participant homes and community settings
• Physical tasks may include personal care and manual handling
