Your key responsibilities include:Providing information about care services and reviewing/plan needs with new and current service usersConducting client intake assessments and personalized care planningReviewing and adjusting care plans based on changing client needs and goalsUndertaking phone assessments based on referrals for new and existing clientsCompleting documentation for intake and assessment protocols using prescribed tools and databasesPromoting seamless and positive experiences by advising on care planningDesired AttributesWe seek a person who can support our community care team to deliver exceptional services.
