About the Role
In this impactful role, you will deliver intensive, strengths-based and trauma-informed support to young people aged 10–17 who are on bail or at risk of entering custody. You will work closely with young people and families across Brisbane to stabilise home life, strengthen relationships and increase positive engagement.
Key Responsibilities
* Provide intensive case management and family partnership support.
* Build strong, culturally responsive relationships with young people and families.
* Support access to education, employment, therapeutic services, housing, health, AOD and other supports.
* Complete assessments, case plans and goal setting in partnership with young people and families.
* Work collaboratively with Youth Justice, police, schools and community agencies.
* Coordinate support teams and assist young people to meet bail or order conditions.
* Provide practical support including transport, advocacy, referrals and appointment coordination.
* Maintain accurate documentation, data entry and reporting to required standards.
