Mai-Wel is the regions leader in NDIS support and employment pathways for individuals in the disability and youth streams. With over 65 years serving our community this is an organisation with people at it's core and delivering for disadvantaged.
The role of Practice Advisor - Complex Case Management is seated in the Employment Pathway of Mai Wel and will be key to providing expertise to the Employment Pathways team. Driving quality of service and maintaining a caseload.
The Role
* Identify need, connect, refer recommendations for identified barriers and social support services
* Collaborate and support teams to develop and implement evidenced based interventions,
* Work in a cross functional capacity across all sites and programs/ contracts,
* Support and mentor regional teams to optimise their performance,
* Provide expert advice on operations, guidelines, and standards, and
* Contribute to business growth and development opportunities.

You will need:
* Experience in human or social services work and knowledge with a strong desire for your background to have experience in employment pathways
* Assess and understand the impacts of interventions for individuals with trauma within the employment pathways
* Specialist knowledge of youth employment services
* Experience regards youth & barriers relating to employment outcomes
* Ability to train, mentor & engage others
* Cert IV or above quals in Disability, Community Services, Mental Health or other relevant stream
* Bring a strong cultural awareness to the workforce
* Ability to analyse data, report and manage accordingly to drive effective operational improvements
* Skilled using MS Office suite
* Always have the participants at the heart of everything you do.
