Care Assess is an innovative, leading provider of assessment services across Tasmania, boasting the provision of assessment services for a number of government agencies. A core function of Care Assess is the provision of Aged Care Assessments in both home and residential settings for the Single Assessment Services on behalf of My Aged Care. Our interdisciplinary team specialises in a wellness and reablement approach to assessments.
We seek an individual with a Registered Nurse or Allied Health professional qualification to join our northern Tasmania assessor team.
The position is full time or a minimum of 4 days per week.
Job Tasks and Responsibilities
• Complete comprehensive assessments with a holistic, clinical, wellness and reablement focus, evaluating client's current abilities, potential to improve and support needs.
• In conjunction with the client, develop a goal focused support plan, addressing the needs of the client while applying a wellness philosophy, to create a quality, person-centred service that enables clients and their carers to live independently in the community.
• Collaborate with other relevant health care professionals to ensure client's needs are being met.
• Facilitate referral to service providers for assessed service needs.

About You
You will have an aged care or health related work history and be an enthusiastic and dedicated individual with a real passion for improving lives. You'll demonstrate the ability to work with limited direction and will thrive in a multidisciplinary environment. You are adaptable and enjoy meeting new people and working in a community setting.
Minimum requirements
• Registered Nurse, Physiotherapist, Occupational Therapist with current AHPRA registration.
• Minimum 2 (two) years' experience in aged care or hospital setting.
• Well-developed communication (written, verbal and listening) and interpersonal skills.
• A demonstrated flexible approach, together with decision making, negotiation and advanced problem-solving skills.
• Understanding of client independence, well-being and a person-centered approach.
• Demonstrated high level computer literacy in word processing, calendar management software and use of client database systems.
• The ability to work autonomously in a geographically dispersed and predominantly mobile team, whilst maintaining a team focus.
• Demonstrated time management and organisational skills.
• A commitment to excellence in customer service with both internal and external stakeholders.
Highly desirable
• Current certification in My Aged Care training.
• Aged care assessment experience
• Demonstrated understanding of service provision, allocation and compliance.
Screening Requirements
• Covid 19 Vaccination Certificate showing evidence of having received two initial doses and the booster.
• Evidence of current flu vaccination.
• Current motor vehicle driver's licence.
• Two (2) recent professional referee checks.
• Satisfactory National Police Certificate obtained within the last three (3) months.
