Working in the social model, our united team of talented change-makers are on a mission to make inclusion possible for all Australians.About The RoleWe are looking for Youth Lead Practitioners to support adolescents and young adults aged 10-24 as they navigate life with a disability, from adolescence through to early adulthood.As a Youth Lead Practitioner at Everyday Independence, no two days will be the same.
If you were here last week, these are some of the things you may have done:Led the interdisciplinary team that plans a young person's NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their familiesUtilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goalsWorked within the social model which means you'll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centresHeld meaningful, future-focused conversations with participants and their supportsEmpowered participants and their families to develop habits and routinesBuilt meaningful relationships with your participants and their supports (family, carers, friends)Documented, and billed for, each session through our online participant management systemAttended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughsYou'll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or supportWho We Are Looking ForAt Everyday Independence, our team thrives on shared values and a team-oriented spirit.
