Join BallyCara as a Social Worker – Part-Time (Wooloowin Location)
BallyCara is seeking a passionate and dedicated Social Worker to join our team! Reporting to the Senior Health Practitioner, you will play a critical role in supporting the social and mental health of our clients. Using your professional expertise, you will guide and inform case management for individuals with multi-faceted and complex needs across a variety of programs.
While this is a part-time role (4 days per week), we are open to negotiating days and starting / finishing hours to suit the right candidate.
Key Responsibilities :
* Provide social work services to older adults, supporting their social and emotional wellbeing.
* Conduct assessments, deliver appropriate interventions, provide counselling, and advocate for clients.
* Collaborate with a multidisciplinary team to develop and deliver proactive, person-centred care.
* Liaise with external service providers and facilitate referrals to ensure clients have access to appropriate community services.
* Support clients navigating the My Aged Care system, including CHSP, HCP, and STRC funding models.
* Work autonomously in the community while contributing to a supportive team environment.

What we're looking for :
* A Degree in Social Work from an accredited institution and membership with the Australian Association of Social Workers (AASW).
* Current Driver's License and a reliable vehicle (with KM reimbursement for work-related travel).
* Strong clinical skills in assessment, intervention, counselling, advocacy, and referral.
* Experience working with older adults, particularly those with chronic and complex health conditions.
* A solid understanding of the My Aged Care system and related funding programs.
* Excellent written and verbal communication skills to engage with clients, carers, colleagues, and external service providers.
* The ability to work independently while collaborating effectively with a multidisciplinary team.
* A commitment to person-centred care and a compassionate, client-focused approach.
* National Police Check (or willingness to obtain one).
