Join Our Team as a Lifestyle Coordinator - Empower Independence and Social Connection for Older Adults
Make a meaningful impact by coordinating activities that enhance the wellbeing, independence, and social engagement of older adults. This role offers the opportunity to work within a supportive, multidisciplinary team, contribute to goal‐oriented programs, and create meaningful participation opportunities that promote empowerment and choice for program participants.
About the Role
As a Lifestyle Coordinator, you will implement, coordinate, and evaluate activities designed to support a productive and fulfilling lifestyle for participants. Using a Participant Directed Care approach, you will provide social support that is goal‐oriented, flexible, and tailored to individual needs, helping participants maintain independence, wellbeing, and meaningful engagement in their community.
You will work collaboratively with a multi‐disciplinary programming team to develop initiatives that meet the diverse social support needs of participants, providing opportunities for social engagement, empowerment, and self‐determination, while operating within program eligibility guidelines and agreed risk parameters.
What We're Looking For
* Completed AQF Certificate III (Level 3) relevant to the position.
* At least 560 hours satisfactory in‐service recent experience within the last 3 years. In‐service experience means service with an agency which includes the same or similar functions to the role.
* Convey strong judgement and initiative in the day‐to‐day execution of a position.
* Demonstrated experience in dealing with aged, frail, or disabled persons.
* Experience in the use of computer packages e.g., Microsoft Word, Excel.
* Demonstrated experience in the provision of a direct care service in a health‐related field.
