Exercise Physiologist Level 3: $2,240.23 - $2,312.82 per week
Dietitian Level 3: $2,240.23 - $2,312.82 per week
Physiotherapist Level 3: $2,240.23 - $2,312.82 per week
Occupational Therapist Level 3: $2,240.23 - $2,312.82 per week
Social Worker Level 3: $2,240.23 - $2,312.82 per week
Psychologist: $1,541.85 - $2,256.76 per week
Clinical Nurse Specialist Grade 2: $2,235.40 - $2,308.80 per week
In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

Join the Aboriginal Chronic Care Program (ACCP), a small but dedicated multidisciplinary team committed to improving the health and wellbeing of Aboriginal people across South Western Sydney. The team includes Aboriginal Health Workers and Practitioners, Care Coordinators, Dietitians, Clinical Nurse Consultants, administration staff, a Clinical Team Leader, Facility Manager and Program Manager, working together across clinic sites at Wellama (Bankstown), Budyari (Miller) and the ACCP offices in Miller. ACCP provides culturally safe, patient-centred care for Aboriginal patients and their families living with chronic and complex health conditions, with a strong focus on holistic wellbeing and mutual support within the team. Through integrated team care and strong partnerships with GPs, Aboriginal Health Services and other healthcare providers, the program supports early detection, education, lifestyle improvement and coordinated care, with the ultimate goal of empowering clients toward self-management and contributing to Closing the Gap in health outcomes. The service spans the Dharawal, Gundungurra and Darug Nations, covering Bankstown, Fairfield, Liverpool, Campbelltown, Camden, the Macarthur region, Wollondilly and Wingecarribee.

You are passionate about supporting Aboriginal patients and their families through culturally safe, respectful and patient-centred care. In this role, you will provide care coordination services for Aboriginal people living with chronic and complex health needs, working collaboratively with an Aboriginal Health Worker to deliver comprehensive assessment, education and care coordination that reflects clinical, psychosocial and cultural considerations. You value teamwork and partnership, building strong relationships with patients, families, carers and service providers to help navigate the health system and improve access to care. You are motivated by making a meaningful difference in Aboriginal health outcomes and thrive in a supportive, community‐focused environment where culture, connection and collaboration are central to care delivery.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

* This is a Targeted Aboriginal and/or Torres Strait Islander Position. If you identify as Aboriginal and/ or Torres Strait Islander, tell us about your mob and connection to culture. If this does not apply to you, please indicate by answering with 'N/A'.
* Current registration with AHPRA as a practicing Registered Nurse, Psychologist, Physiotherapist or Occupational Therapist or professional qualifications in Social Work, Exercise Physiology or Dietetics which provides eligibility to the relevant professional association. CNS Grade 2: Relevant post-registration qualifications and at least 3 years' experience working in the clinical area of the employees specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
* How has your experience delivering culturally safe chronic and complex care to Aboriginal communities shaped your understanding of the factors influencing their health, and how do you apply health coaching and goal-setting techniques to support patients with complex needs?
* Can you describe how your communication skills, critical thinking, and collaborative approach with patients, carers, and multidisciplinary teams contribute to responsive care delivery, while also supporting risk management, quality improvement, and workplace safety?
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
