Job Title: Pharmacist - Slade Pharmacy, St. Andrew'S Toowoomba Hospital
This is a challenging role that requires a skilled pharmacist to work across both retail and hospital pharmacy settings.
Main Responsibilities:
1. Provide efficient clinical pharmacy services in a clinical area
2. Conduct medication history interviews, reconciliation, and reviews at transfers of care
3. Dispense and distribute medication effectively
4. Monitor therapeutic drug reactions and outcomes
5. Offer medication counseling and education to patients
6. Collaborate with multidisciplinary teams and provide advice on patient care
7. Participate in training and educational sessions for pharmacy staff and other healthcare professionals
8. Support quality use of medicines through projects, audits, and committees
About the Role
The successful candidate will have at least 1-year of practice experience and be registered as a pharmacist. They will work closely with other healthcare professionals to deliver high-quality pharmacy services to patients and customers alike.
