Job Title: Disability Support Practitioner

About Our Team:
Our shared values are Passionate, Respectful, Ethical, Curious and Courageous. We have an exciting position in our Accommodation and Respite in Great Western Sydney region for a dedicated individual who supports clients through compassionate care and empowerment.
Your Key Responsibilities:
* Support clients to develop and maintain independence and advocate for their individual needs.
* Deliver personalised support by assisting clients at mealtimes, personal care, administering medication, manual handling using equipment and aids.
* Transport clients in wheelchair accessible vans and pool cars.
* Monitor clients' well-being and report on their progress.
* Be part of a fun and dynamic team.
Requirements:
* Demonstrated commitment to supporting and promoting individual client needs.
* Flexibility to work on a 24/7 rotating roster, including morning, afternoon, and night shifts, including weekend and public holiday shifts.
* Experience supporting customers with intellectual disability, high physical support needs, and medical/health needs.
* A current NSW Driver's License (P2 minimum).
* Local to the North Shore.
