Job Description:
The Atherton Rd Community Mental Health Service is a multidisciplinary team of professionals working together to provide comprehensive mental health care.
Our team includes Occupational Therapists, Social Workers, Registered Psychiatric Nurses, and Clinicians such as Consultant Psychiatrists, Junior Doctors, Patient Transition Co-ordinators, and Clinical Psychologists.
We aim to provide patient-centred care that considers all aspects of a person's needs, encouraging the support and involvement of carers or family members.
The role of Case Manager involves working collaboratively with other clinicians to ensure high-quality care is provided to consumers in the service area.
As a Case Manager, you will provide comprehensive mental health assessments and demonstrate competence in managing complex mental health issues while adjusting to changing priorities.
The ideal candidate will be an innovative thinker, passionate about achieving excellence, and have a supportive disposition.
They will be able to collaborate with their team to reach goals and achieve great outcomes for mental health consumers.
