**About Us**

Buddies Community Access Network (C.A.N) is a local organisation, established in 2017, providing NDIS Core Support services in and around the Ballarat area. Buddies C.A.N operates differently to other disability providers. We understand that in life, not everyone is suited to each other. We take the time to carefully pair the right 'Buddie' to our participants, to achieve the best outcome with their NDIS goals.

**The Role**

The Intake & Engagement Officer is responsible for participant intake and assessment tasks, as well as support planning, monitoring, engagement, and review support. They are responsible for tracking participant NDIS budgets and plan spends, ensuring the supports being provided best meet their needs and goals and liaise with Buddies staff, Support Coordinators, LAC's, therapists, and informal supports when needed.

.**Key Responsibilities**
- Intake of new referrals and participants
- Completing filing, scanning, answering phone enquiries, setting up intake appointments, conduct meetings with participants and their families, data entry using spreadsheets and data bases to ensure all data is collated and filed efficiently on Brevity).
- Ensure participants are supported to identify, choose, and manage their own daily and lifestyle routines
- Complete a check in to review and update Support Plan documents at 3 months and 9 months and making them accessible to support staff.
- Monitor plan spends and ensure funds are being maximized and participants are engaged with relevant programs/services.
- Maintain effective internal and external stakeholder relationships

**About You**

You are passionate about working in the disability sector, creating programs to empower our participants to achieve their goals and to pursue their dreams. You will have the chance to make a difference to the lives of our participants, in a fun, goal focused way.

**Skills and Experience**
- Qualifications and/or experience in managing NDIS participants intake, engagement and care planning processes is preferred but not mandatory
- Cert III or IV Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar is preferred but not mandatory
- A good balance between administration and customer service experience
- Efficiency using Microsoft Programs and able to pick up internal systems quickly
- Adaptable to embrace change and use your initiative to find more efficient ways of working
- Strong networking and relationship building skills
- Proven ability to think and plan strategically with excellent analytical and problem-solving skills

**Mandatory Requirements**
- Current WWCC (employment)
- NDIS Screening checks
- Valid driver's license and comprehensively insured vehicle
