Care Advisor (Entry or Experienced Level)1 x position located at Glen Waverley office – Permanent Part time (0.6 EFT)1 x position located in Morwell office – Permanent full time (1.0 EFT)$39.2424 to $43.0771 per hourWhat you’ll be doing in this role?We take a wellness and enablement approach to care under the consumer-directed care model that also takes into account client capacity. Care Advisors work with a diverse range of clients with Home Care Packages of all levels. Care management levels will vary depending on the level of support chosen by the client. This role involves facilitating client choice in decision-making and meeting needs using analytical processes that require planning and a goals-orientated approach.This role involves travelling to meet clients in the community as well as working in an LCHS office.
