Now Hiring: Full-Time Home Care Package Care Partner
Location: Hervey Bay | Mon–Fri, 8:00am – 4:30pm
Optional 4-day work week – enjoy a long weekend every second week
Make a real difference in your local community. Just Better Care is seeking a dedicated Clinical or Non-Clinical Care Partner to join our Hervey Bay team. In this community-based role, you'll provide personalised support to older Australians, helping them live safely and independently at home.

Key Responsibilities
* Complete in-home assessments and care planning
* Manage participant cases and provide clinical/holistic support
* Track participant wellbeing and document progress and goals
* Liaise with medical professionals, allied health, and service providers
* Maintain documentation to ensure compliance (CRM, funding, evidence uploads)
* Oversee participant budgets and ensure funding guidelines are met
* Support training and guidance of support workers and nurses
* Schedule and coordinate services, including urgent changes
* Contribute to ongoing audits, reporting, and professional development
* (Clinical Care Partners only) Provide occasional wound care
What You'll Need
* Advanced computer & CRM skills
* Strong administration, organisation, and multitasking abilities
* Excellent communication and customer service skills
* Ability to work both independently and in a team
* Knowledge of wellness, reablement, and enablement approaches
* Current First Aid/CPR
* Valid Police Check, NDIS Worker Screening Card & WWCC
* Current Australian Driver's Licence
* Experience in home/community/aged care
* Knowledge of Home Care Package guidelines & compliance
* Current AHPRA registration (RN or EN)
