In this pivotal position, you will play a key role in ensuring continuity of care and maintaining the highest standards across each patient's individual care journey. You will coordinate complex admissions and lead comprehensive discharge planning for patients admitted to our statewide Spinal Injuries Unit, supporting seamless transitions and exceptional clinical outcomes.

The Role

As a Clinical Care Coordinator—Clinical Nurse Specialist Grade 2, you will maintain continuity of care and facilitate high standards throughout the patient journey. You will coordinate complex admissions and discharge planning within Prince of Wales Hospital's statewide spinal injuries service, providing individualised, case-managed complex patient care and leading the spinal injuries unit.
Your role includes leadership of an interdisciplinary team, ensuring people experience person-centred, safe, high-quality compassionate care, while managing autonomy of decision-making and advancing clinical practice.
Key Functions
Complex admission and discharge planning of patients within the statewide spinal injuries unit
Collaborate with the spinal injuries MDT team
Attend admission intake and discharge planning meetings
Coordinate with the patient flow team and update on the spinal waitlist, ensuring smooth patient flow between facilities
Engage patients, families or carers by providing specific spinal support and education
Where You'll be Working
Prince of Wales Hospital and Community Health Service is a Level 6 tertiary referral hospital with 370 inpatient beds. Each year it cares for more than 70,000 Emergency Department patients and around 50,000 inpatient admissions, offering outpatient and rural outreach services as well as innovative virtual models of care. The hospital provides an exceptional staff experience and enhanced patient and family outcomes through person-centred care.

Selection Criteria
Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
Relevant post-registration qualifications and at least 3 years' experience in spinal injury or spinal rehabilitation (or equivalent)
Ability to manage complex clinical care and service delivery issues and solve problems using clinical management plans
Extended autonomy of decision making within multidisciplinary teams while observing professional and ethical boundaries
Skills in patient and carer education and mentoring nursing staff for professional development
High-level written and verbal communication, interpersonal skills, and ability to identify key messages and concerns
Experience leading and initiating activities to improve clinical service delivery and patient outcomes
Competence in using health-related information technology
