What You'll Be Doing
Lead and support Quality Use of Medicines (QUM) projects and initiatives across the LHD.
Analyse medication usage data, identify trends, risks, and opportunities for improvement.
Develop, review, and implement medication-related policies and procedures.
Engage with clinicians, managers, and stakeholders to promote safe and effective use of medicines.
Provide backfill support for the Voluntary Assisted Dying (VAD) Pharmacist (up to 0.2 FTE).
Participate in rollout of enhanced digital patient record and electronic medication prescribing platform.
Work collaboratively with Pharmacy Educator and other team members.
Qualifications and Experience
Registered pharmacist with AHPRA.
Experience in hospital clinical pharmacy.
Strong communication and stakeholder engagement skills.
Ability to work independently and in a team.
Experience or willingness to work with the Voluntary Assisted Dying (VAD) team.
Compliance with Department of Health criteria for Grade 3 pharmacist.
