Role Summary:
We are seeking a Physiotherapist to join our team and be involved in providing community-based care programs that assist aged customers to continue living independently in their own homes.
The successful candidate will have a customer base in the North Metropolitan area and have access to pool cars or paid mileage, as well as a laptop and phone.
This is a full-time position working 37.5 hours per week with no shift work required, with work hours Monday to Friday during core business hours.
Key Responsibilities:
* Conduct comprehensive assessments and develop personalized care plans
* Design individual and group exercise programs to optimize client function and independence
* Assess and provide solutions that support client autonomy and well-being through equipment assessment and prescription
* Foster collaboration and successful outcomes by communicating effectively with clients, families, and caregivers
* Ensure seamless care coordination and accurate progress tracking through meticulous record-keeping
* Work closely and collaboratively with the multidisciplinary team to achieve desired outcomes for clients
Requirements:
* Current FULL registration with AHPRA as a Physiotherapist (essential)
* Ability to relate to elderly customers with empathy while working effectively with other health professionals and service providers
* Excellent interpersonal and communication skills
* Proven ability to assess, plan, implement, and evaluate quality care
* Current Australian driver's license and reliable vehicle - both essential
