**Who are we looking for?**:
We are seeking an energetic and highly motivated person who is passionate about providing holistic care to join our Health and Wellness team as a Clinical Coordinator. This role is a fast paced position, centred around the assessment and implementation of rehabilitation plans for people returning to their homes after a hospital stay.
You will find yourself working with our customers to create, implement & case manage a short term rehabilitation plan by engaging Allied Health Practitioners & other health care professionals. This is an integral role which empowers our customers to recover in their own homes & ensures they will receive appropriate care.
This position is best suited to any Allied Health Professional registered with AHPRA including: Speech Pathologists, Dieticians, Social Workers, Occupational Therapists, Physiotherapists and Nurses.
**What can you expect to be doing?**:
- Meet with customers across Adelaide Metro to assess and create rehabilitation plans.
- On-going case management of customers throughout the program.
- Engage and arrange the appropriate Allied Health Practitioners & Health Care Professionals for your customers.
- Provide treatment to customers based on your clinical background and expertise as a part of rehabilitation plans.
- Work collaboratively with AnglicareSA's broader Health & Wellness team
**What do you need to bring?**:
- Current AHPRA registration or background
- Proven ability to be flexible, adaptable & able to work collaboratively within a multi-disciplinary team
- Excellent communication skills, with the ability to build rapport with customers and stakeholders quickly
