Role Overview:
As an Enrolled Nurse / Client Coordinator, you will be responsible for providing high-quality clinical care to clients while also coordinating services and ensuring efficient client management. You will work closely with a multidisciplinary team to support the ongoing health and well-being of clients, ensuring that all clinical and administrative needs are met in a timely and professional manner. Applicant must be willing to travel to the surrounding areas.
Key Responsibilities:
Provide nursing care to clients, ensuring their safety, comfort, and well-being in accordance with clinical guidelines and best practices.
Act as a liaison between clients, families, healthcare professionals, and external service providers.
Conduct client assessments and reviews to ensure care plans are effective and adjusted as necessary.
Maintain accurate and up-to-date client records, documenting clinical observations and treatment plans.
Ensure compliance with healthcare standards, policies, and regulations.
Participate in the development of care strategies tailored to individual client needs.
Provide education and support to clients, families and carers on managing care at home or within a clinical setting.
Contribute to the continuous improvement of service delivery and client satisfaction.

Qualifications
* Enrolled Nurse (EN) with 1-2 years of experience in community nursing and current AHPRA registration.
* Alternatively, a Certificate III or IV in Aged/Disability Care with a minimum of 4 years' experience.
* Proven experience in aged care, disability, or mental health within a community setting.
* Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
* Current First Aid, CPR certification, and National Police Check.
* Working With Children (WWC) check or willingness to obtain.
* A valid driver's license and access to a reliable car with comprehensive insurance.
Preferred Skills
* A dynamic professional with strong communication skills and a friendly, professional telephone manner.
* Skilled in critical thinking, problem-solving, and multitasking under pressure with attention to detail.
* Experienced in networking, building relationships, and establishing new referral sources.
* Exceptional customer service and proficient in verbal and written communication.
* In-depth knowledge of home care services, industry guidelines, and best practices.
* Skilled in wound care, catheter care, and PEG feeding techniques.
* Strong understanding of NDIS services, policies, and guidelines.
* Excellent time management and organizational skills
* High level of computer literacy and proficiency with relevant software
