Moonlight Care is dedicated to providing high-quality, person-centered care for individuals with disabilities within the NDIS framework. We are currently seeking a passionate and skilled Registered Nurse to join our team in Perth Metro as a full-time Clinical Care Coordinator.
As our Clinical Care Coordinator, you will be instrumental in upholding the highest standards of care for our NDIS participants. Your responsibilities will include:
* Developing and implementing individualised care plans: Create and oversee person-centered care plans that directly address the unique needs of each participant.
* Ensuring clinical safety and compliance: Manage clinical risk, ensure adherence to NDIS guidelines, and promote a safe environment for participants.
* Empowering our support team: Provide essential training and ongoing support to staff on medication management and best practices for participant safety.
* Fostering collaborative care: Work closely with healthcare professionals and other support services to achieve optimal outcomes for our participants.
* Managing clinical incidents: Thoroughly investigate, accurately document, and effectively respond to any clinical incidents that may arise.
* Driving continuous improvement: Implement quarterly strategies to enhance our services and proactively identify areas for improvement.
* Contributing to quality leadership: Collaborate closely with the Moonlight Care leadership team to champion quality care initiatives and improvements across the organization.
* Maintaining meticulous records: Ensure all records and documentation are accurate and compliant with NDIS requirements.
We are looking for someone who is:
* A Registered Nurse (RN) with current registration.
* Experienced in a clinical setting and within the disability sector, ideally with direct NDIS experience.
* A strong leader with excellent communication and problem-solving abilities.
* Deeply committed to delivering exceptional, participant-focused care.
* Professional verbal and written communication skills
* High level of organisational skills
* Sound computer skills in Office and Excel
