Job Opportunity
Your role as a Customer Service Coordinator will involve managing a portfolio of clients requiring community-based care services.
You will assess client needs and develop individualized Care Plans, manage client funding while adhering to government program guidelines, and onboard new clients while proactively engaging existing ones using a supportive approach.
* Main responsibilities include:
* Client case management for individuals living in their own homes
* Care planning and budgeting for community-based aged care services
* Collaboration with internal teams to deliver comprehensive support
* Onboarding new clients and fostering strong client relationships
Required Skills and Qualifications
* Experience in community-based case management is beneficial
* Strong communication skills, both verbal and written
* Demonstrated empathy and compassion
* Excellent problem-solving abilities and critical thinking skills
* Cultural competence and the ability to work with diverse populations
