Working as part of a team across Morphett Vale and the surrounding areas, you will enjoy autonomy in carrying out your work, traveling to aged care homes to conduct AN-ACC assessments.
While direct care isn't part of the day-to-day work, you will use your expert clinical skills and knowledge to conduct interviews with residents and staff and review health records to inform the AN-ACC assessment.

What you bring:
* 5 years' experience in aged care or a related area of health care, including working with people with dementia.
* Demonstrated experience in aged care including working with people with dementia.
* Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration.
* Able to conduct clinical assessments using a range of assessment tools.
* Strong organisational skills and the ability to work independently and as part of a team.
* A genuine desire to improve the lives of older Australians.
* Your own vehicle as travel will be required within your region.
