• Develop and implement personalized care plans tailored to individual client needs
• Coordinate and manage care services to ensure the highest standards of care
• Foster strong relationships with clients, stakeholders, and team members while assisting clients in meeting their unique needs and goals
• Liaise with clients, families, and healthcare professionals to ensure comprehensive care
Requirements:
• Experience in case management and client care coordination in a Community Aged Care setting
• Strong verbal, written, and interpersonal communication skills to communicate and work effectively with a broad range of people
• Vaccination against Influenza & at least 1 dose of Covid-19 vaccine
• Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
• Current Police Screening Certificate
• First Aid and CPR Certificate
• NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
• Current Queensland driver's licence
• Tertiary qualifications and/or relevant experience in a health-related field
